This section contains the different forms used in our practice to register clients and patients when they visit to Emerson Animal Hospital.  They keep the doctors and staff informed of your pet's health and your contact information as we need it. 

Please print the necessary forms from this website after discussing your planned visit with a doctor or one of our receptionists, complete it in the comfort of your home, and bring the completed form with you when you bring your pet for its appointment.  Completion of this paper work at home saves our clients time at the clinic and avoids too long a stay in the reception room where there are often a variety of other animals, both large and small, a sometimes exciting situation.


Notice

There are two methods of obtaining a printed copy of the forms below for completion at home.  Each is easy and convenient.

First, each form that must have a signature is available for printing through Adobe Reader, a program available on many computers.  If your computer does not already have this option, the necessary software may be downloaded from the Adobe website by clicking the logo below and following the instructions.  Once this is done, find the form you desire, click the Click Here for Printable Client Registration Form link and the form will appear, then, click Print.

A second method is done by "highlighting" the desired text on the computer screen (left-click your mouse and drag the cursor over the text to be printed, then, release) and clicking Print. Both give good copies of the form which may be completed in the relaxed atmosphere of your own home, rather in the hectic surroundings of our animal hospital.
